Waiver of Liability Forms

An “Acknowledgement of Risk and Waiver of Liability” form is used to:

  • Inform an individual of potential risks involved in an activity

  • Allow an individual to voluntarily choose to incur the risk

  • Secure an agreement from the individual to hold harmless LCSC

Waiver forms are useful to participants because they alert participants to higher risk activities,and when more caution should be displayed.  Waiver forms alert the participant to situations where Lewis-Clark State College liability ends and personal liability begins. 

For an effective waiver, it should be tailor made to each activity or program.  If you need a new waiver please request one through Administrative Services at 208-792-2240 or email. Below is a sampling of waivers used by the college.
