Waiver of Liability Forms

An “Acknowledgement of Risk and Waiver of Liability” form is used to:

  • Inform an individual of potential risks involved in an activity

  • Allow an individual to voluntarily choose to incur the risk

  • Secure an agreement from the individual to hold harmless LCSC

These forms are frequently used in both personal and business transactions.  Waiver forms are useful to participants because they alert participants  to higher risk activities and when more caution should be displayed.  Waiver forms alert the participant  to situations where LCSC liability ends and personal liability begins. 

If you are offering an activity or event that is a higher risk (ex: shooting classes, bow and arrow classes, rock climbing, etc.) please notify the Administrative Services office to determine if a waiver is needed.
