Affirmative Action Process


The Affirmative Action Packet must be completed for all benefit eligible positions at Lewis-Clark State College.


1.


New Position Approval Form:  Complete this form for all new positions and route for the necessary signatures prior to the search.  If this is a Classified position, a Position Description Questionnaire also needs to be completed.
   
2.


Academic Affairs Only:  Authorization to Conduct a Search in Academic Affairs:  Complete this form for all positions reporting to Academic Affairs and route for the Dean and Provost signatures prior to the search.
   
3.


Request to Hire, Advertisement Plan, Job Announcement, Job Description, and Search Committee Members:  Complete these forms with the necessary position information and route for the required signatures.

Academic Affairs Only:  Permission to Interview Candidates:  Complete this form for all positions reporting to Academic Affairs and route for the necessary signatures prior to inviting candidates onto campus for an interview.

Disclosure & Authorization Form:  Final applicants for each position will need to complete this form to allow Human Resource Services to request a background check.  The background check will be done only on the successful candidate.  However, if that candidate is ultimately excluded because of the information found in the report, the hiring authority can request a check for another applicant in the final pool.

Report of Applicant Pool:  Complete this form and route to Human Resource Services before an offer of employment may be extended to a candidate.

Academic Affairs Only:  Permission to Make a Verbal Job Offer:  Complete this form for all positions reporting to Academic Affairs and route for the necessary signatures prior to offering the position to a candidate.  All salary offers must be pre-approved by the Provost before negotiations with a potential employee.
   
8.

Report of Hiring:  Complete this form as soon as the background check is complete and the candidate accepts the position and route to Human Resource Services.
   
9.

Personnel Action Form:  Complete this form after the candidate has accepted the position and before the candidate begins employment in order for Human Resource Services to set up the employee in payroll.
   
10.

New Hire Paperwork:  Schedule an appointment with the candidate and Human Resource Services, 792-2269, to go over the necessary benefit paperwork.
