Application Materials
On this page you will find the documents needed for application to the A.S. Radiographic Science Program.
Read the first several pages of the document carefully. This is where admission criteria and processes are explained. Pay particular attention to the application window, program prerequisites, entrance testing and GPA and other admission requirements.
Submit your completed application to the Division of Nursing & Health Sciences. Professional references (when required) must include both pages, and should be hand delivered, mailed, or faxed to the Division office by the individual completing the form (references should not be submitted by the applicant). You will be contacted if there is a problem with your application. If you have questions, contact your academic advisor or the Nursing & Health Sciences Advising Center (
