The Office of Disability Services will assist students with disabilities who self-identify and meet LCSC criteria for eligibility to receive reasonable and appropriate accommodations, academic adjustments, and/or auxiliary aids determined on a case-by-case basis. Students may print and complete the documentation below. Call 208-792-2211 to set up an appointment with the coordinator for disability services.  You may fax (208-792-2453) before appointment or bring in the completed paperwork at the time of your appointment.  Also bring in appropriate documentation of your disability.
