Documentation Verification Forms

To ensure the provision of reasonable and appropriate services for students, the Office of Disability Services requires students to provide current and comprehensive documentation of their disability and its impact on their education.

Select and print the form required for verification of student's disability.  It should be taken to student's evaluator for completion and then sent or taken to the Office of Disability Services at LCSC, Reid Hall 111.  Fax: 208-792-2453.

Attention Deficit/Hyperactivity Disorder
(ADHD) Combined Type, Predominately Inattentive Type, or Predominately Hyperactive-Impulsive Type.
Should be filled out by a licensed psychologist, neuro-psychologist, psychiatrist, or other relevantly trained medical doctor, or counseling professional.

Learning Disabilities
Should be completed by a licensed psychologist, neuro-psychologist, psychiatrist, or other relevantly trained medical doctor.

Psychological and/or Psychiatric Disorders
Should be completed by a licensed psychologist, neuro-psychologist, psychiatrist, or other relevantly trained medical doctor or counseling professional.

Pervasive Developmental Disorder (Autism, Aspergers)
Should be completed by a licensed psychologist, neuro-psychologist, psychiatrist, or other relevantly trained medical doctor.

Physical Sensory
Should be completed by a licensed physician, psychologist, audiologist, ophthalmologist, or other licensed professional appropriately qualified to diagnose the specific disability of the student.
