Provider Visit: $45
                     Nursing Visit: $15

Additional charges for laboratory, radiology, pharmacy and medical procedures will be added to the price of the office visit.

New to the 2010 Policy at Student Health Services Only:
Paid at 100%. Limited to one test per policy year. Includes but not limited to the following:
Preventative Care: Includes Woman's Wellness Exams, Sexually              Transmitted Disease (STD) Screening, Annual Blood Work and Prostate Screenings.

At the time of scheduled appointments, the Student Health Services - Technical Records Specialist II (SHS-TRSII) will collected all co-pays and the student's portion after insurance. If covered by the Student Health Insurance Plan (SHIP) claims will be submitted by the SHS-TRSII.

Student Health Services does not bill private insurance. Patients with outside insurance will be responsible to pay at time of service and may personally submit a claim for reimbursement.
