Meal Plan Selection Form 2005-2006

Please select one of the following options. The meal plan you select will be charged to your LCSC account. For prices, please refer to the rate sheet.

	Platinum -	110 All You Care to Eat Meals and $400 Flex
	Diamond -	7 All You Care to Eat Meals Per Week and $300 Flex
	Gold -	75 All You Care to Eat Meals and $375 Flex
	Commuter	- 16 All You Care to Eat Meals and $500 Flex (Seaport Only)

All you care to eat meals- All you can eat meals can be applied weekly toward dinner Mon - Sun and brunch on Sat. or Sun.

Flex money- Can be used throughout the semester as needed.

Note: If a meal plan is not selected or this form is not returned, the Diamond Plan will be charged to your account.
