Replacement WarriorOne Cards:

• 1^st Card- No Charge
   
• Replacement Card- $10 – this includes lost cards, etc.
   
• Name Change Replacement Card- No Charge – this includes name changes due to marriage or divorce.  All other name changes are subject to the replacement card fee.
   
• Damaged Card- $5 – this includes cards damaged due to wear or incident; must present card or replacement fee applies.
   
• Defective Card- No Charge- this includes cards that are not damaged and do not read correctly in campus card swipes; testing of the card is required to receive no charge for a replacement.
   
• Stolen Card- No Charge- to receive a no charge for a stolen card, you must provide a campus security report or a police report verifying that you experienced a theft.  The replacement card charge applies if no report is provided.
   
• Aged Card- No Charge- students and staff can request a new card, free of charge, every five years.
