Make sure the Database window is displayed and that the Form
Objects are displayed. |

Click the  button. In a few moments, the New Form dialog box will display. |

Double-click Form Wizard. The Form
Wizard will start. |

Double-click the name of each field
you want to include on your form. |

Click the Next > button. |
|
Choose a layout from the list. |
|
Click the Next > button. |
|
Choose a style from the list. |
|
Click the Next > button. |
|
If necessary, type a name for your
form. |
|
If necessary, click a view option. Example: Modify the form’s design |
|
Click the Finish button. |
