
Certificates of Insurance - LC State
A certificate of insurance (COI) shows what type of insurance an entity has. When someone requests a COI from LC State, they want proof of our coverage. When LC State requests a COI from third parties, it is to ensure that state requirements are met when we contract with vendors and construction companies.
LC State Proof of Insurance: A certificate of insurance (COI) that shows coverage for LC State will be limited to $500,000 per our Retained Risk Program through the State of Idaho. LC State is not able to "Additionally Insure" third parties, but can add as "Certificate Holder."
LC State participates in the Retained Risk Program administered through the State of Idaho and is subject to the limits of liability in Idaho Code 6-901 through 6-929, known as the Idaho Tort Claims Act (ITCA). The Retained Risk Program is self-funded for liability exposures and privately insured for property losses.

Professional Student Liability Insurance: This COI can be requested by emailing risk@lcsc.edu. This insurance is purchased separately and is not a part of the Retained Risk Program.
Request a Certificate of Insurance
To request a Certificate of Insurance, fill out and submit the State Risk Liability/Auto/Property COI Request Form. The normal turnaround time for certificates is two or three days. Please allow at least one week to process certificate requests.
Instructions for Requesting a COI
- Requesting Agency: Lewis-Clark State College (Always)
- State Personnel Initiating Request: Enter your name

- Type of Certificate Needed: Click the boxes that apply to your event or activity
- One time request or annually renewed certificate? Indicate if this request is needed every year or just once
- Certificate Holder: Enter the name of the entity requesting a certificate
- Address of Certificate Holder: Enter the entity's address
- ATTN TO: Enter the name of the individual contact at the entity
- Job, Location or Contract/Ref# if required to be noted on Certificate: Enter this information if it applies
- Choose what type of situation this certificate is for: Choose the appropriate activity from the drop down menu
- Comments: Include any clarifying information regarding reason for needing the Certificate of Insurance
- Miscellaneous Supporting Documents: Upload agreement, contract, or other supporting documents if available
Once completed, hit "Submit." You will receive an email confirmation.
Certificates of Insurance - 3rd Parties
LC State requires proof of insurance from third parties (companies, organizations, vendors, contractors, agencies, or other groups) who use campus facilities for events or who provide services to the college.
To assure appropriate insurance is in place to protect the college, email the Insurance Requirements form to to the third party, who can give it to their insurance agent.
LC State's insurance requirements are:
Insured shall maintain (CGL) coverage with limits of not less than $1,000,000 each occurrence and $2,000,000 aggregate.
If the use of a personal or company owned vehicle is used in the scope of their business, insured shall maintain auto liability coverage with limits of not less than $1,000,000 for each accident.
Worker's Compensation coverage must meet all statutory requirements.
All policies, except Workers’ Compensation, shall name Lewis-Clark State College, 500 8th Avenue, Lewiston, Idaho 83501 as an additional insured. A copy of the Additional Insured Endorsement must be attached to the certificate verifying that the insured’s policy(s) has been endorsed as required.
